MAM root-cause fix: indexer 12 was 403 "mam_id expired" / auto-disabled since 14:20 — Prowlarr + refresh-timer shared ONE rotating mam_id and desynced. Fix = Justin created a DEDICATED Prowlarr MAM session; installed new mam_id via /api/v1/indexer/12?forceSave=true, re-enabled, verified live results (Old Man 363 seed, Vonnegut EPUB coll 678, full Hollis catalog).
